It is hereby clarified that this questionnaire, in all its parts and entirety without exception, is the property of Dr. Oren Kaholi. No copies made of this questionnaire or any part of it are allowed in any way and in any media, without the prior written consent signed by Dr. Oren Kaholi.
PRAIUM Medical Solutions diagnoses and treats the root causes of any medical complaint and not merely the symptoms. It considers all bodily systems and organs as one whole unit, taking into account not just their functions, but also the way they influence and interact with each other. Moreover, every person is an individual and no set of conditions and circumstances related to one patient is identical to another’s, even when both are suffering from the same category or definition of a medical pathology. PRAIUM Medical Solution adapts the treatment to the individual causes and circumstances of each patient.
According to the diagnosis findings, a personalized treatment strategy is prepared to suit the specific patient. This includes PRAIUM Medical Solutions’ advice and recommendations as to the estimated duration of the treatment process, the frequency of the treatment sessions and the estimated various formulas and other materials needed for the first phase of the treatment (which is aimed at stopping the deterioration, stabilizing the patient and embarking on a recovery process).
Everyone has his/her own unique physical constitution, mental disposition, personal and medical history, personality, habits, lifestyle, diet and way of functioning in the world. All these unique variables have a direct impact on the individual healing process – i.e., the degree of success that can realistically and reasonably be expected and the duration of the treatment process.
Therefore, the success of the treatment and its duration cannot be foreseen in advance for certain. The actual sessions’ frequency, as well as the formulas and other materials needed can only be determined during the medical process itself, as certain unpredictable conditions may occur – e.g. patients who react differently to the treatment, the accuracy in which the patient implements the medical advice given to
him/her by the clinic staff, new urgent medical needs evolving during the period of the treatment, and any other change effecting the planned treatment process.
In this way, the initial treatment strategy may be changed and adapted during the process according to the patient’s needs.
The medical treatment provided by PRAIUM Medical Solutions includes a variety of means and tools, such as SNT (“Single Needle Technique”) unique acupuncture combined with patent purely herbal formulas produced exclusively by PRAIUM Medical Solutions, as well as other materials provided according to need for enhancing the treatment process. The treatment process also includes the clinic’s advice for a wise nutrition, life style habits supporting health and the right amount and kind of physical exercise, adapted to the patient’s condition.
PRAIUM Medical Solutions and its staff have the sole discretion not to accept for treatment any person, especially in cases where according to the clinic staff estimation, that person might not benefit from its services for any reason.
PRAIUM Medical Solutions and its staff have the sole discretion to stop the entire treatment process in any of these cases:
- When no progress has been recorded by the clinic or reported by the patient.
- The patient’s lack of compliance with the clinic medical advice and instructions, which might hinder the treatment process;
- Patient concealing vital information that can affect the treatment’s estimated success;
- Patient providing any false and/or misleading information;
- Inappropriate behavior;
- Patient takes or uses other kinds of materials, medicines, medical procedures and/or advice, which might hinder the treatment process.
- A payment was not made in full 21 days after providing the service for which the payment was due or 21 days after a “cancellation in debit” has occurred, as defined below. Stopping the treatment process does not annul the right to collect the debt.
- A breach of any other obligation by the patient (including those taken according to this ” PATIENT INFORMATION” and/or “PATIENT CONCENT” forms);
- Other circumstanced that may hinder the efficiency of the process and its estimated results.
It is hereby clarified, that PRAIUM Medical Solutions’ doctor is not an MD. Any diagnosis and/or treatment provided by PRAIUM Medical Solutions does not replace or contradict in any way opinions, diagnosis, medical assertions, follow up checks and any medical treatment recommended by the patient’s MDs. The patient bears the sole responsibility for consulting with his/her MDs and whether to act upon their advice. If the applicant / patient is on any conventional medication, he/she bears the responsibility of consulting with his/her MD about the necessity of continuing the medication prescribed by the MD and using their own discretion.
Any PRAIUM Medical Solutions diagnosis and/or treatment (sessions, formulas, other materials and advice) do not contradict or change in any way any advice given to the patient by his/her MD doctors, including any medications, check-ups, tests, procedures, etc.
When the patient’s condition improves to the point of being able to gradually reduce or stop any medication prescribed by an MD doctor and/or when a contra-indication is found between the prescribed conventional medications, it is the sole responsibility of the patient to consult his/her MD doctors.
The order of the clinic’s appointments and the diagnosis analysis are determined according to the medical urgency and not necessarily according to the order of the applications. The clinic is doing its best to serve all the patients scheduled with the outmost efficiency and set timetable. However, there might be cases where a treatment session is prolonged or urgent cases arrive, which according to the medical discretion of the staff must be treated first without a prior appointment. We ask for our patients’ patience and understanding in those occurrences, knowing that when their turn comes, they would be given the same utmost attention needed for their medical needs.
Clinical experience shows that in some particular cases the treatment process might be temporarily accompanied by a phenomenon known as a “healing crisis”. When this sometimes happens, the patient may experience a sort of aggravation of existing symptoms or some new symptoms, such as headache, rash, inflammation, pain, heat or cold sensation, weakness, fatigue, “flu-like” symptoms, acute diarrhea etc. Even though this involves a certain amount of discomfort, the body has various ways to rehabilitate its natural inner balance and some might present themselves temporarily as an illness or a disorder; so these symptoms should be understood as the bodily natural ways of retaining again an eventual state of health and harmony.
Therefore, if any sort of a “healing crisis” is experienced by the patient at any point of time during the treatment process, he/she should immediately notify the clinic therapist about it and receive the clinic’s help for it. PRAIUM Medical Solutions strongly advises against resorting to the old ways of dealing with such symptoms, which did not succeed in the past and have led the patient to turn to PRIAUM Medical Solutions help in the first place. The patient is, of course, always free to choose his/her way of dealing with these symptoms, but without a prior notification of the clinic and the advice of its therapist, it might become impossible to continue with the treatment plan, or any.
The patient is solely responsible for arriving to the treatment session on time. Arriving late might shorten the session. Being late 30 minutes or more will be considered as a non-show and the patient will have to cover the full costs of the session. Any treatment cancelled less than 24 hours prior to the scheduled appointment for reasons that do not constitute “force majeure” or in case of a no-show, the patient will be also billed with the full price of the aborted treatment session. (All the instances mentioned in this paragraph are defined as “”cancellations in debit”
For the efficiency and treatment ease, the patient is requested to maintain personal hygiene – especially before arriving to a treatment session. The patient is requested to arrive with comfortable and fitting (not tight) cloths, neither hungry nor thirsty.
The formulas and any other products purchased or provided in PRAIUM Medical Solutions’ clinic are not refundable in any way, even if they were not used or the treatment was stopped.
The price of a treatment session includes the SNT needling treatment only. The needling session price does not include an aesthetic treatment (which is priced separately) and does not include any product of any kind, such as formulas in any form, creams, anointments, drops, sea salt, oils, amino acids, minerals, vitamins, other supplements, etc.
Purchasing in advance a whole treatment series, will award the patient with the discounts set by the clinic, provided that the payment is settled fully in advance. There are no refunds for any cancelled and/or non-show treatment sessions even if they were included in the treatment series.
Any payments not made in full and on time will result in a cancellation of all reductions of any kind given from the regular clinic prices.
